Opening a mail box

To open a mailbox, please, print and fill out the following forms: PS1583 – please, fill it out but DO NOT sign it! You must sign this form when you are at the store.
Reminder: You must have two forms of ID to open a mailbox account. Please refer to FAQ to find out what forms of ID are acceptable.

Mailbox Service Agreement

Optional Services

The following forms are for optional services: ACH Debit Authorization: If you wish to set up automatic payments to be withdrawn directly from your checking account. Please attach a voided check to this form.

Credit Card Charge Authorization: If you wish to set up automatic payments charged to your credit card. Have this card ready to show at the store.

Notification Agreement: If you wish to be notified that you have mail through an e-mail or text.

Mail Forwarding Agreement: if you wish your mail to be forwarded.
